Hello Bridge Player!  All eligible individuals who enter Ohr Kodesh Congregation are required to be fully vaccinated (and boosted) for COVID-19 unless they have a documented medical exemption.  Proof of vaccination or medical exemption is required in order to enter the building.  

To facilitate the entry process, Ohr Kodesh is issuing an OKC VaxPass to individuals who submit proof of vaccination or medical exemption.  Individuals can present the OKC VaxPass in order to enter the building.

This form allows you to upload images of your documentation (and those of your family/friends) so that an OKC VaxPass can be mailed for each individual.  For security purposes, Ohr Kodesh will not keep your COVID-19 Proof of Vaccination or Medical Exemption Letter on file. We will delete the image of your upload within 48 hours of your form submission.

For those with Medical Exemption letters, we require 48 hours to review and approve copies of those letter in order to issue an OKC VaxPass, which will be your only means of entry.  We will not be able to review Medical Exemption letters for 'day of entry' access.  You must have an OKC VaxPass for building access.

Once proof of vaccination has been submitted, we still require you to show proof of vaccination or your OKC VaxPass each time you enter the building.  Using your OKC VaxPass will speed up your entry.
