ECC Summer Discovery Registration

The OKECC Summer Discovery program for 2023 will take place in three, three-week sessions from 9:15am-1:00pm. This program serves children who are at least two years old and have not yet begun kindergarten.

The Ohr Kodesh Early Childhood Center Summer Discovery 2023 Registration and Policies document outlines the core program options, dates and fees, registration deadlines and payments, emergency and medical forms, financial policies, and placement and notification . 

Your registration will be complete after this form is submitted along with a $50 non-refundable registration charge per child, per session.

Which of the following best describes your child's connection to the Ohr Kodesh ECC? (This question tells us which forms we already have on file.) 

To qualify for member program rates,* families must be OKC members in good standing during each session their child is attending. Families with membership status for the 2022-2023 period are eligible for the member program rate during session 1. Families with membership status for the 2023-2024 period are eligible for the member program rate during session 2 and session 3.

*Membership status will be verified with OKC administration. Any changes in status that differ from the choices indicated below may result in an adjustment in the fees owed.
Core Program 9:15 am - 1:00 pm

Families may enroll their child(ren) for any one, any two or all three sessions. Individual weeks may not be prorated.

2-3's M-W-F: 
These sessions are for children ages 2 & 3.

*Children must be age 2 by June 1, 2022 to attend camp.*
For incoming or current ECC students: The 2-3's M-W-F sessions are for students enrolled in the 2s program for the 2023-2024 school year and children currently enrolled in Half Day 2s (M-W-F) class for 2022-2023. 

3-4's M-F: 
These sessions are for students ages 3 & 4.

For incoming or current ECC students: These sessions are for students enrolled in the 4s program for the 2023-2024 school year and children currently enrolled in the Half Day 3s class for 2022-2023. 

4-5's M-F: 
These sessions are for students ages 4 & 5.

For incoming or current ECC students: These sessions are for students going to kindergarten for the 2023-2024 school year and children currently enrolled in the Half Day 4s class for 2022-2023.

I hereby apply for my child to attend Ohr Kodesh Congregation’s Early Childhood Center Summer Discovery Program. I understand that my financial commitment for the Summer Program and the options to stay later on days with Enrichment Activities extends to all costs incurred from June 12, 2023 through August 11, 2023.
I understand that following my $50 non-refundable application fee submitted with this application, my full tuition fees are due on or by May 5, 2023.
I am aware that Summer Discovery tuition will include an additional security fee of $25 per family, per session.
I have read the document entitled Ohr Kodesh Early Childhood Center Summer Discovery 2023 Registration and Policies, which includes the “Financial Policies,” and the document entitled Ohr Kodesh Early Summer Discovery 2023 Fees. I agree to be bound by the terms stated in both of these documents.
I am aware that Ohr Kodesh Congregation has a COVID-19 Vaccine Mandate requiring all adults and children who enter Ohr Kodesh Congregation to be fully vaccinated for COVID-19 unless th­ey have a documented medical exemption.

My child is fully vaccinated for COVID-19.

I have reviewed the current Ohr Kodesh ECC Respiratory Virus Protocols (Updated 1/29/23)
I have read the portions of the registration cover letter entitled Required Emergency and Health Forms. I understand that my child may not attend the summer program, regardless of payment of fees, without proper completion and submission of the applicable documentation in accordance with the Maryland State Department of Education Office of Child Care.
I have read a copy of the consumer education brochure entitled Parent’s Guide to Regulated Childcare.
I authorize school officials, in case of emergency, to request that the Rescue Squad transport my child to the nearest medical facility for treatment.
I give permission for my child to accompany the class on walking field trips near the school.
I give permission for staff to re-apply a non-prescription over-the-counter sunscreen after the initial outside playtime. I will administer the first application of the day before camp. I will provide non-expired commercial sunscreen in its original container, labeled with my child’s name. 
Use of Images: Photographs, images, and/or video taken of ECC students are used within the ECC and broader Ohr Kodesh community. This includes but is not limited to classroom and hallway displays, printed materials such as brochures and newsletters, the OKC monthly newsletter, the OKECC website and OKECC social media. As a policy, we do not use children's names in conjunction with any video or digital images that are shared in OKECC social media. I agree to the use of images for this child.
