On this page you will find information and required forms related to applying, registering, and enrolling your child(ren) at Ohr Kodesh Early Childhood Center for the current school year.

Each January, we post the necessary application information and forms for the subsequent school year. INFORMATION FOR THE 2020-2021 SCHOOL YEAR IS NOW AVAILABLE! Please use the links below to print the applicable forms for the current school year.  

2020-2021 REGISTRATION INFORMATION & APPLICATION FORMS

The following forms provide you with all the necessary information to register your child for the 2020-2021 school year.

  • OKECC Family Handbook 2020 - 2021 -  Please note the content of the the Reopening Guidelines takes precedent over the OKECC Family Handbook. It is published solely for the communication with members of Ohr Kodesh Early Childhood Center. Its use for any other purpose is not authorized. Replacement or additional copies may be purchased from the school.
  • A Parent's Guide To Regulated Child Care - Note: In lieu of submitting the signature page from this brochure, please initial the applicable statement on the online registration form as indication that you have been provided a copy of the brochure. A hard copy is available upon request.

Enrollment Forms

Prior to your child's first day of school, Maryland State Department of Education and the Office Of Child Care require the following forms be on file at our school. All enrollment forms for 2020-2021 should be submitted by August 1, 2020. These forms must be completed every school year and for each child enrolled. Please note, Maryland licensing periodically updates the forms. Completed forms submitted prior to an update remain valid, but any old versions submitted after an update will not be accepted. Therefore, please be sure to check this website for the most recent form before filling out or forwarding to your doctor.  

Forms are valid for one year from the signature date and will need to be renewed if they expire prior to the end of the 2019-2020 school year (e.g., a Health Inventory dated February 2019, will expire in February 2020, and therefore an updated form would be required for attendance through the remainder of the school year). (NOTE: If your child is due for a wellness visit between August 1, 2020, and August 31, 2020, then please contact us.)

  • 2020-2021 Emergency Form (This information is collected from the registration form that is completed when a family registers via the OKECC Online Family Database and no additional paperwork is required.)
  • 2020-2021 Health Inventory Part I, Part II, and Blood Lead Testing Certificate:
  1. Part I is to be completed and signed by a parent.
  2. Part II must be signed by your child’s doctor, regardless of whether the medical office opts to provide a computer printout of the required information. If your child's doctor attaches a printout, then it must include all of the information requested on the original form and must be signed, dated and stamped by the doctor's office.
  3. Blood Lead Testing Certificate to be completed and signed by a parent and/or doctor. Only if all answers to Section A are "No" is a parent signature sufficient; otherwise, your child's doctor must complete and sign the form.  
  • 2020-2021 Immunization Chart (Doctor offices may complete the information directly on the form or provide a printout on office letterhead. Regardless of the format used, all documents require the doctor's signature, date and stamp.)
  • 2020-2021 Medication Administration Authorization Form (A separate form must be completed for each medicine to be kept on site. A doctor's signature is required on each form for all prescription medicines. All prescription and nonprescription medicines must be in the original container with any labels intact and be within the current use date. Prescription medicines must also have the pharmacy label intact. Note: Basic care items, such as sunscreen, insect repellent, and diaper ointment, may be authorized via your child’s online account/registration form.)
  • 2020-2021 Allergy Action Form and Asthma Alert Form (To be completed and signed by both a parent and doctor, as applicable to each form. These forms are required for any child with a listed allergy or asthma condition noted on their Health Inventory and/or their Medical Administration Authorization Form. If your child requires medication to be kept at the school please see directions above for Medication Administration Authorization Form.)

Early Drop-off

We offer Early Drop-off and Extended Afternoon options to all children enrolled in our Half Day program, as well as various Enrichment Activities for both Half Day and Full Day children enrolled in our age 3/4's and age 4/5's classes. Families can enroll for these program options for the entire school year or on a drop-in basis (with 24-hr advance sign-up and if space is available). The following forms provide more details regarding the program options for the current school year.
