B'nai Mitzvah Forms

B'nai Mitzvah Reservation Form                                                                            Please click on the link above and complete the form to secure your B'nai Mitzvah Date. A $500 deposit (a portion of the $1500 OKC B'nai Mitzvah) is required at the time of submission.

Reception Reservation Form                                                                                            If you are planning a reception at the synagogue in conjunction with your B'nei Mitzvah at any time other than immediately following the service, this form must be completed and submitted with the required $200 deposit. 

The date for your reception cannot be guaranteed unless both the form and the deposit are received by the OKC Office.
